Choose "Ship to Billing Address" to ship to billing address on file.  You can edit Buyer Name and Email Address fields for shipping purposes only.  Order confirmation will be sent to the defined email address.  If any information is incorrect request a Billing Information Change under your Account Options or call customer service at 1-888-898-5623 .

Choose "Ship to Another Address" if you would like to ship to a location other than your billing address.  All fields are definable. Order confirmation will be sent to the defined email address.

Once your order is submitted, a purchase order will be created for you. The purchase subtotal price excludes any applicable tax and shipping.
